Dental Sleep Medicine for Sleep-Disordered Breathing

Dental sleep medicine is a specialization of dentistry that focuses on treating sleep-disordered breathing by using oral appliances, such as the Vivos DNA or mRNA appliance. These appliances are worn in the mouth to correct jaw alignment and open up your airway.

Sleep-disordered breathing
is an umbrella term to describe any medical condition that affects your ability to breathe easily while you are asleep. One of the most common of these conditions is sleep apnea, which can affect your overall health and quality of sleep.

What Is Sleep Apnea?

"Apnea" comes from the Greek word "apnos," meaning "breathless." Sleep apnea is the term for conditions that affect your ability to breathe properly throughout the night. In essence, it means "breathless sleep."

There are three main types of sleep apnea, but they all have one factor in common: they cause your body to stop breathing sporadically throughout the night. This urges your brain to send signals to wake your body up so you breathe again, which can happen dozens of times per hour.

When you are constantly waking up throughout the night, the result is poor quality of sleep, which can affect your ability to function during the day. If left untreated, sleep apnea can negatively impact your health and lead to other serious medical complications.

The Three Main Types of Sleep Apnea:

Obstructive Sleep Apnea

With obstructive sleep apnea (OSA), the muscles in the back of your throat relax too much when you go to sleep. Those muscles keep your airway open so you can breathe properly. If they relax too much, they obstruct (or close off) your airway, causing you to choke or gasp awake. OSA is commonly treated with a continuous positive airway pressure (CPAP) machine, which uses air pressure to keep your airway open while you sleep.

Central Sleep Apnea

With central sleep apnea, your respiratory muscles (such as your diaphragm and chest muscles) stop functioning when you are asleep. These muscles are designed to run automatically by your central nervous system, hence the term "central sleep apnea." But when the signals don't arrive, or when your respiratory muscles ignore the signals, your breathing stops. This makes your brain wake your body up throughout the night to restart your breathing.

Complex Sleep Apnea

Also known as treatment-emergent central sleep apnea, this third type is caused by treating obstructive sleep apnea. What happens is that your OSA gets treated, but then your body develops central sleep apnea. It is not exactly known how or why this happens, but it has been observed in sleep apnea patients who use CPAP machines to treat their OSA. However, there may be other contributing factors that lead to complex sleep apnea, as well.

Sleep Apnea Symptoms

The most common sleep apnea symptoms are as follows:

  • Loud snoring
  • Waking up gasping, choking, or snorting
  • Headaches or migraines
  • Poor focus, memory, or concentration
  • Lethargy or exhaustion
  • Dry mouth or sore throat in the morning
  • Mood changes
  • Sudden short temper or irritability
  • Loss of interest in sex
  • Insomnia
  • Temporomandibular Joint disorders (the joints where your jaw connects to your skull, in front of your ears)

Some of these symptoms, such as loud snoring or waking up multiple times per night, may be hard for you to notice on your own. That's why it's important to pay attention to the more obvious symptoms listed above that you
can notice yourself.

If you've been experiencing the above symptoms, you should schedule a consultation with Dr. Maryam Seifi. Even if you aren't certain you have sleep apnea, Dr. Seifi can help you determine if you should see a sleep specialist for an official diagnosis.

How Untreated Sleep Apnea Can Impact Your Health

If you are diagnosed with sleep apnea, it's very important to keep that old saying in mind: "An ounce of prevention is worth a pound of cure." Delaying your treatment can result in more serious medical complications down the line, such as:

  • High blood pressure
  • Liver problems
  • Heart problems (heart attack, stroke)
  • Type 2 diabetes
  • Metabolic syndrome

Childhood Sleep Apnea

Sleep apnea can also affect children. Unfortunately, the vast majority of children with sleep apnea are misdiagnosed with learning disabilities and behavioral disorders. By trying to treat issues with learning or behavior, well-intentioned parents miss the real cause of their child's troubles—that they are exhausted from lack of good quality sleep due to a lack of oxygen getting to the brain.

Children can have the same symptoms of sleep apnea as adults, such as migraines, mood changes, and exhaustion. Some of the symptoms that are more common in children with sleep apnea are:

  • Trouble wetting the bed
  • Underweight or stunted growth
  • Trouble focusing in school
  • Behavioral issues
  • Chronic or repeated illnesses

If you are worried about your child, you can take our helpful assessment to determine whether they may be affected by sleep apnea. You can also schedule an in-person consultation with Dr. Maryam Seifi by calling Breath of Life Dental at (301) 818-2653.

How the Vivos System Improves Sleep Apnea

The Vivos System is a nonsurgical treatment that treats obstructive sleep apnea using a custom-fitted oral appliance. This appliance will gently change the shape and position of your jaw to correct alignment, size, and shape. Vivos appliances can relieve obstructive sleep apnea, snoring, or TMJ dysfunction, depending on your current health needs.

Both Vivos oral appliances are removable, and they are designed to be easy to put on or take off yourself. In general, you will need to wear your Vivos appliance for 12–15 hours per day to achieve the desired results.

The appliances look like retainers, such as what you might wear to keep your teeth straight after getting your braces taken off. This makes them more discreet than other sleep apnea treatment options, which many patients enjoy.

The Two Types of Appliances

  • DNA (the Daytime-Nighttime Appliance)

The Vivos DNA appliance for sleep apnea expands your palate and reshapes your jaw, which increases the size of your airway and allows you to breathe easily all night long. When the obstructions are gone, your obstructive sleep apnea goes with it, and you can get the restorative sleep you need for a healthy life.

The DNA appliance fits to your upper jaw and your lower jaw. It is meant to be worn at daytime and nighttime, and your total treatment length will be between 12–24 months. When your treatment is finished, you will no longer need to wear the Vivos DNA appliance.

  • mRNA (the mandibular Repositioning Nighttime Appliance)

The Vivos mRNA appliance for obstructive sleep apnea shifts your lower jaw forward to properly align it with your upper jaw. It also helps expand your upper jaw and hard palate to increase the size of your airway.

This is also an excellent treatment for temporomandibular joint (TMJ) dysfunction. By shifting your lower jaw forward, the Vivos mRNA appliance relieves stress on the TMJ and improves function in your jaw.

The mRNA appliance is designed to be comfortable because it is meant to be worn at nighttime. Your total treatment length will be between 12–24 months. When your treatment is finished, you will no longer need to wear the Vivos mRNA appliance.

What Do the Vivos DNA and mRNA Appliances Treat?

Dr. Seifi has successfully used the DNA appliance and mRNA appliance to treat a variety of conditions in her patients, including:

  • Chronic snoring
